Follow these steps to register:

1) Purchase or renew your US Lacrosse membership 

    (Contact us if you have questions)

 

2) Have your health insurance information handy

 

3) Click the 'registration link' below and complete the fields.  Be certain

    to download a copy of the Athlete Acknowledgement Form

 

4) Print and fill out the Athlete Acknowledgement Form and hand it

    to your coach

 

5) Welcome to the team!
